Please ensure you meet the qualifications below prior to filling out any of the required information. You must also meet USA Hockey Requirements to be rostered.
Must be a veteran of any military service, either Active Duty, Reserves or National Guard, with a discharge under honorable conditions.
Must not be currently on Active Duty. (Exceptions apply if your official separation date is within 3 months).
Must possess a disability rating of 10% or more, or a purple heart.
